RA in teeth and jaw? Options

Dear All

I have woken up today with an awful pain in my teeth and jaw on both sides. I am a regular attendee at the dentist so I do not think it is a problem with my teeth as such. I presume you can get RA in your teeth and jaw? I have also had a bad pain at the base of my skull at the back for the last few days.

I am unable to open my mouth very far and am finding it difficult to talk (much to my Husband's amusement) and to eat.

I presume I just have to wait for it to pass?

Hi Jackie

Sorry to hear that you are suffering with your teeth and jaw. I have had problems with my jaw in the past and I know how painful it can be. Since starting on Enbrel my symptoms have settled down and I have not had problems with my jaw at all but like you I seem to always be at the dentist mainly because of inflamed gums which seem to be a weak area of mine with a lowered immune system.

I hope that your symptoms subside soon and wish you a pain free weekend.

I'm sure I've read that RA can affect any joint in the body. I only had it in the jaw once and that was during a crippling flare up. I can remember having trouble opening my mouth and it really hurt to do so. I don't know about the teeth but I know I get sore gums sometimes which I put down to the mtx. I hope your pain lessens soon. Naomi, X

I had a problem with my jaw about a year ago very similar to yours. I could hardly open my mouth to speak ( much to the delight of hubbySmile) or eat ( except chocolate) and spoke to my rheummy about it. He said that RA can affect the jaw and when he touched it he said it was swollen and was inflamed. It settled down again.Over the last few weeks it's started up again,accompanied by horrible grinding in the joint -different from the last time, I van open my mouth and no swelling I spoke to my dentist this time and she said it was probably due to the fact that I grind my teeth at night (stress!). I have one of those mouthguard things which I wear at night, and it is helping.

It might be worth talking to the dntist and the rheummy.

forgot to mention- I had it in the back of my head this time too. That's settled but the jaw still hurts.

Hi Jackie

I have had awful problems with my jaw for several years on and off and have quite a bit of damage on both sides. The jaw is a joint so unfortunately it is affected by the dreaded R.A. Inflammation in the jaw can be one of the most difficult things to cope with because you can't open your mouth properly. Whenever I go to the dentist he is very understanding because I can't open my mouth very wide or for long.

Have you had an x-ray on your jaw to see if there is any damage? A few tricks for managing jaw pain, stay away from hard and crunchy food or anything that requires alot of force to chew, no toffee lol!!! Also if there is alot of inflammation going on I find an ice pack on the side of the face helps as well as rubbing peppermint oil on. Also lots of face pulling (at your husband)Laugh to strengthen the facial muscles (if it's not too painful).

You may need a drug assessment if there is active inflammation, do you take anti-inflammatories, arcoxia always helps my jaw pain.

The pain is a little easier today. I did not sleep very well at all last night as my hips are also giving me jip

About five weeks ago I went for my normal Methotrexate blood test and it flashed up on the computer I had not had my blood pressure take this year and therefore the nurse said it needed to be done. He did it, and did it again and did it again. He said my blood pressure was really high for me, normally 120/90 but was 150/102. He said it could just be I was a bit stressed or because I had been rushing from work. He told me to have it done again at my next blood test.

This morning I went along for my blood test thinking at least the inflammation I am experiencing in my joints will be shown on the blood test for the RA team to look at. Had my blood pressure taken, and taken again, and taken again. It is now 160/102. The nurse said I had better book in and see the doctor as soon as possible. So I went out to Reception and nearly fell over in shock when they offered me an appointment for tonight.

Duly arrived at the doctors tonight and went in to see a lovely Scottish lady doctor (first time I had seen her). She took my blood pressure on one arm and then on the other arm and the blood pressure was still the same as this morning. She said I could just be worked up because I had been told my blood pressure was high. I mentioned the jaw pain and she said it could either be connected to the high blood pressure or the RA. Anyway I now have a blood pressure monitor to test my blood pressure four times a day over the next two weeks. If it continues to be elevated then she will talk to me about medication.

Whilst I was there I mentioned that I had been feeling very uncomfortable "down below". I have previously had two prolapse bowel and bladder operations and the last time the surgeon "trussed me up" he said they would never fall down again. I thought maybe I had piles (not something I had ever had before). The doctor told me to hop on the couch and she would have a look. After a long examination both front and back she told me to get dressed. When I sat down again she said the good news is that I do not have piles .............. the bad news is that I have a prolapsed bowel and bladder again.ThumbDown. I had to have a hysterectomy 15 years ago, the year after my Daughter was born, as I had a very complicated birth which caused a lot of damage. Without the womb my bowel and bladder have nothing to support them. Because I have already been operated on twice, the third round would be very complicated. I am going to stay with the discomfort for as long as I can as I cannot face any more surgery at the moment.

So the week started with a pain in my jaw and has gone downhill from there. Really the pain in the jaw is nothing compared to what else is now going on.

Rebecca - in answer to your question on medication, I am on Methotrexate, Naproxen and the anti-tnf Simponi. The doctor does wonder if the Simponi is also causing high blood pressure, so again this may need to be looked into. I have been on the Simponi for about six months (having failed on three other anti-tnf's) and it worked ok in the beginning but I am flaring more as the months go by. I am due to go back and see the RA Consultant in February and really there is not a lot else they can offer me as I had a massive reaction when they gave me a Rituximab infusion and I have been through all the ant-tnf's.

On the bright side, it is the weekend and I am going to put my feet up and not think about health.

Hi Jackie - you have had a really tough week, and I really feel for you. I think you need lots of tlc and time to rest and come to terms with all this news. Enjoy your weekend.

It does sound as if your RA has flared up and is causing various knock on effects - I've had the jaw thing and once the inflammation is under control it'll probably just disappear by itself - try to focus on not grinding teeth and do some gentle exercises to relieve any buildup of stress. It must be so distressing to face the thought of more surgery, but you know that you've got through it before, so can do again! Be strong and get yourself feeling as good as possible before you face it. And about the high BP - mine was really high even before the RA and my GP kept nagging me. In those days I poopooed it, and resented taking any meds, but now I take bendroflumethazide and lisinipril daily, plus I have a more healthy diet and weight, and am delighted that my BP is well within the normal range. It's good that you've spotted the problem before it causes anything serious, so now you have time to do something about it

What a rough time you are having Jackie!!!

My aunt came off anti-tnf because it was causing high blood pressure, you need some guidance and advice from your rheumatologist on that one in February. It's a shame you are not seeing him sooner.

Also have you tried Tocilizumab, that's the next option after anti-tnf and Rituximab?

Hope things start to improve for you soon.

Jackie just read the post and thought o my gosh! What more is this poor woman going to have to put up with?
Glad the GP saw you quickly
My bp goes up when I'm in pain
But the teeth pain etc might be hormone deficiency related- worth having thyroid checked.

Jaw- certainly had trouble with this and the RA was all doing it.
Found rubbing phorpain gel on my cheek and behind my ear soothed the pain a bit
Got this on prescription

I do have a friend who had both sides of jaw replaced. It looked shocking to read about etc but you'd never know to look at my friend now

You can have steroid injections in the jaw apparently too

Hi Jackie, just to say, sorry you are having such a rough time with everything that is going on.
Not very imp to you now perhaps when you have so much else to contend with, but just to say that I do get a lot of tooth pain in my front teeth, what I call neuralgia type pain. Mostly happens when I have been out in the cold and then come in the warm, and it does hurt. Seems to happen a lot since having ra.

Gosh Jackie, what a rough time you're having.
I'm another with jaw problems thanks to the RA so I do empathise there.
Is there any chance you can get your rheumy appointment brought forward?
As Rebecca says, Tocilizumab could be an option for you perhaps? I'm due to start it next week.

Thank you for all your lovely replies.

I think this RA is being rather mischevious at the moment playing hide and seek. The jaw pain has almost gone. I did not sleep last night for pain in both hips and down my legs. I am not sure if it is RA or muscle pain. Managed to haul myself out of bed and into work this morning but was in terrible pain. I sat for most of the morning and suddenly by lunchtime it had gone. So what was all that about?

I am going to try and last until February before seeing the Rheumy team if I can and let the Simponi work a bit more. With all the anti-tnf's I have tried I have had to stop them in days or a few weeks due to severe reactions but this time I have no reaction (unless it is pushing up the blood pressure).

My pain has not really been under control recently and I do wonder if this has also not helped the blood pressure.

If the jaw pain returns I will ask for a prescription of phorpain gel. Thanks for the advice Jenni.

When I go to bed at night I do wonder which part of my body is going to cause problems tomorrow but I think that is part and parcel of RA.
